This session will focus on Care Model Redesign and how it can support your work. We will recap on how Population Health can help you understand your local priorities and how Population Health Management (PHM) can help you identify your priority segments and cohorts. We will then focus on how you might understand the needs and preferences of your cohorts and identify the care functions, workforce and estate (and other enablers) requirements to meet their needs. The following areas will be discussed and examples will be provided.

The session will cover:

  • Prevention (targeted and universal)
  • Long Term Conditions (single and multiple), including group consultations
  • Complex needs (and MDT working)
  • Same day demand (the big challenge at the moment!)

    Steve Laitner

    Steve Laitner is a practicing GP in St Albans, Hertfordshire and a freelance health consultant. After public health and clinical leadership roles in the NHS at local, regional and national levels he moved into the world of healthcare consulting.

    Steven supports a number of NHS organisations, healthcare providers and health-tech start-ups with their clinical strategies, partnerships and delivery.

    Given his clinical and public health foundations Steve has a particular interest and expertise in Population Health Management (PHM) and has recently supported the NHS England Place Development Programme using PHM approaches.

    He also has a passion for shared decision making (previously the national clinical lead), long term conditions, patient empowerment, patient leadership and service redesign. He also has a particular interest in patient access and brought in “telephone triage” into his practice about 15 years ago.
