The West Kent Medicines Optimisation Team would like to invite primary care colleagues to an education event on: Care Homes Best Practice.

Acacia Sooklal Lead Integrated Care Pharmacist and Swapna Thummala, Senior Integrated Care Pharmacist will be the guest speakers at this event and will be delivering a fantastic session to support PCN colleagues with care home related matters.
