Care Model Redesign

February 1 @ 1:30 pm - 2:30 pm

This online session will focus on Care Model Redesign and how it can support your work. We will recap on how Population Health can help you understand your local priorities and how Population Health Management (PHM) can help you identify your priority segments and cohorts. We will then focus on how you might understand the needs and preferences of your cohorts and identify the care functions, workforce and estate (and other enablers) requirements to meet their needs.
