This session will cover all you need to know about Population Health Management (PHM) and how it can support your work. We will cover the essential elements of PHM, both in terms of the data and atypical elements, and also the service redesign which needs to follow.

We will use real examples from primary care across the NHS and show how you can use the incredible data which sits in primary care and across the health and care sector to deliver more personalised, proactive care (and better reactive care too) to better meet the needs of different groups in out communities
