Our Highest Needs cohort constitutes almost 90% adults over 65 with high complexity with associated frailty needs as well as small number of younger cohort, including children & young people, who would all benefit from Comprehensive Holistic Assessment principles. Please join us to discuss the MDT shared approach to share our collective learning to how we assess them across the proportionate KMCR supported e-forms to maximise our shared care records.

Please join many of our system clinical experts for each of the webinar focus areas and please join the vibrant Q&As on the chat line for each session.

2 sessions: Tuesday 29th September 7-8.30pm or Thursday 1st October 12noon-1pm

Join our webinar to explore Structured Medication Reviews (SMR) and learn how these can support safer, more personalised medicines management for people with complex health needs.

As people live longer with multiple long-term conditions, medicines optimisation and regular medication review are increasingly important.

SMRs provide an opportunity to take a holistic view of a person’s medicines, ensuring that treatment remains appropriate, effective and aligned with their needs and preferences. By supporting collaborative working across health and care settings, SMRs can help improve outcomes, reduce medicines-related harm and enhance the patient experience.

Join either of our two webinars (both sessions contain the same content) for practical guidance, demonstrations and the opportunity to ask questions.

Who Should Attend?

Health and care professionals involved in medicines optimisation, prescribing, medication reviews and personalised care planning, including colleagues working in primary care, community services, acute trusts, care homes, hospices and wider health and care settings.

Book your place on the SMR webinar on ONE of the two sessions available (evening or lunchtime).
