Lower Limb Wound CARE Programme Modules:

The programme modules will include:

1.    A Population Health based approach to improving wound care outcomes in your community – Setting the scene for understanding the complexity of wound care as a population health issue

2.    Identifying need and creating better joined up car

3.    Influencing change

4.    Why effective integrated team working is essential

5.    Leadership and creating the momentum for change

6.    Innovation in Practice

The programme will focus on the following outcomes:

1.      Standardise care

2.      Improve outcomes

3.      Demonstrate needs of a holistic patient centered approach including the wider determinants of population health

4.      Enhance Holistic Patient Support

The interactive sessions will seek to improve the holistic care of this patient population group. All the participants will be expected to identify, develop and deliver an innovation project and complete a case study. This can vary from managing a patient with a specific condition to enhancing service improvement and will be supported by the CARE programme team.

The programme will be delivered virtually by expert facilitators on Monday 30th March 2026 and Tuesday 31 March 09:00 – 12:00.
