Kent and Medway ICB is supporting General Practice to adopt the Johns Hopkins risk stratification tool, including the use of Patient Need Groups (PNGs), to enable surgeries to proactively identify and support patients with complex needs, enhance continuity of care, reduce avoidable hospital admissions, and improve QOF outcomes.
