The Kent and Medway Primary Care Training Hub are pleased to announce an exciting opportunity for a part time Cardiometabolic Fellowship supporting the Sustainable Healthcare Unit (SHCU). The Fellowship is open to multiprofessionals working within Kent and Medway Primary Care who have a keen interest in cardiovascular disease (CVD) prevention and cardiometabolic conditions, as well as experience in leadership and delivering and embedding Quality Improvement Projects within their practice or PCN.
The Fellowship placement is for one year commencing in late September/early October 2025 and will be for 2-4 sessions a week working on a project to support the SHCU. There is no academic programme aligned to this Fellowship; however the successful applicant will gain experience of delivering and embedding change to help improve health outcomes for residents within Kent and Medway and the work will help to build a successful Integrated Care System (ICS).
