Trainee Pharmacist Supervisor Registration Form

This form must be completed by all Designated Supervisors and/or Designated Prescribing Practitioners for Foundation Trainee Pharmacists.
Please forward to any colleagues who will be taking on the role of Designated Supervisor or Designated Prescribing Practitioner for the 2025/26 Foundation Trainees. If you do not have a trainee starting in 2025, you do not need to complete this form.
Please note: to complete the form successfully the supervisors will be required to know the trainee’s name, oriel pin and the confirmed trainee start date. You also need the GPhC employment site number.
All trainees are required to have a named Designated Supervisor prior to starting their programme as described in the training site requirements.
Information on the trainee, their programme and their designated supervisors is required to be shared with the GPhC two weeks prior to their start date. Please be aware that e-portfolio access for trainee pharmacists will be dependent on Designated Supervisor allocation; failure to submit information in accordance with NHS England’s timelines, will result in a delay in provision of access.
Please endeavour to submit Designated Supervisor details at least 3 weeks prior to the trainee starting their training.
All trainees on the full GPhC learning outcomes are required to have Designated Prescribing Practitioner (DPP) to supervise a period of learning in practice relating to prescribing during the FTPP.
Designated Prescribing Practitioners details must be submitted at least 3 weeks prior to commencement of their prescribing learning in practice within the programme. All trainees are required to have a named Designated Prescribing Practitioner by week 13 of the programme.
If you have not secured a DPP please contact your regional pharmacy team as soon as possible.
Any changes to supervisors (both Designated Supervisors and Designated Prescribing Practitioner) within the FTPP must be reported via the FTPP Change Form *coming soon*. Any new supervisors starting within the FTPP will be required to complete a registration form.
It should be noted that where the trainee pharmacist has a multi-sector rotation (of 13 weeks or more) into another organisation, there must be another named Designated Supervisor or a Designated Prescribing Practitioner, at the rotational site.
